The Calgary COPD & Asthma Program provides free comprehensive respiratory education for patients with COPD, asthma, or chronic cough and smokers at risk.

The 45- to 60-minute education sessions are one-to-one by phone or Zoom with a Certified Respiratory Educator who can:

  • Review and observe use of medications
  • Develop asthma or COPD action plans
  • Help develop strategies to reduce/eliminate tobacco or cannabis use and more

The educators ensure patients are provided with the highest quality care and respiratory education that can lead to:

  • Improved management of their condition
  • Enhanced quality of life

CWC PCN team members have used the Calgary COPD and Asthma program when they have patients who are struggling with asthma or COPD and are not improving sufficiently.

Eligibility

Patients 16 years or older who meet one of the following criteria:

  • Present with signs or symptoms of asthma, COPD, or chronic cough that have not been diagnosed with a respiratory condition
  • Have been diagnosed with asthma, COPD, chronic cough where symptoms persist and/or the patient has experienced exacerbations of their respiratory condition in the past year
  • Had a recent admission to hospital or emergency department due to exacerbation of respiratory condition

Complete the Calgary COPD & Asthma Program respiratory education referral form and fax to 403.476.7772.
