All members can refer their patients for no-cost 24-hour blood pressure monitoring to their choice of four providers with six locations using our single, combined referral form.

How to refer

To refer, please complete the referral form in your EMR — search for “CWC PCN 24h BP Referral Form” — (or download the PDF referral form) and fax it to the location of your choice:

• 24-hour blood pressure monitoring with no cost for your patients
• A report back to you with interpretation of the results
• Additional capacity to meet your need for 24-hour BP monitoring
• The ability to book urgent referrals when indicated

Twenty-four-hour blood pressure monitoring provides a clear picture of how blood pressure changes as patients move through their day. It can rule out the “white coat effect,” monitor the effects of medicine on blood pressure, or identify if blood pressure stays high at night.
