Our PCN designs and implements programs and services that link with community-based partners and support members to build and sustain Patient’s Medical Homes (PMHs). By providing an array of helpful programs and services, we can make a difference in the lives of family physicians so they can make an even more significant difference in the lives of their patients.

Service eligibility and brief description

Access Appointment Service:

  • For your patients who need medical care within 24 hours but are unable to obtain an appointment with you, and for unattached patients within our PCN’s area (inclusion-exclusion criteria).
  • Open every day to offer temporary support in place of a walk-in clinic or ER, a summary of your patient’s visit after the appointment, and after-hours coverage.

Chronic Disease Management (Enhanced and Comprehensive members):

  • For patients who need support to address a variety of concerns, including hypertension, heart health, diabetes, obesity and weight management, COPD, asthma, smoking cessation, various gastrointestinal issues, and many other conditions.
  • Our Primary Care Registered Nurses are trained in many areas related to chronic disease and are certified in HealthChange Methodology.

Mental Health Program:

  • Provides patients a phone consultation with a Primary Care Registered Psychologist (PCRP) who works with the patient to help them decide the best care pathway for their unique needs: referral to short-term psychotherapy with a PCRP (patients of Enhanced or Comprehensive members only) or to a mental health service in the community.
  • Accepts both minor (typically 12 years old and up) and adult patients with the full list of referral reasons on the referral form.
  • For pediatric patients, a direct referral to The Summit — for patients living in west Calgary — or Eastside Community Mental Health Services — for patients living in east Calgary — is recommended.

Senior Services:

  • For patients who are 65 years or older or physiologically aged who would benefit from integrated, team-based consultative care.
  • A specialized team — geriatric nurses, geriatric physicians, geriatric psychiatrists, and pharmacist — can complete in-depth assessments that may include cognitive screening, functional assessment, environmental risk assessment, medication review, fall assessment, or nutritional assessment and act as an advocate for patients.

Social Workers:

  • For families and patients of all ages who need short-term assistance accessing support and navigating the health system.
  • The program accepts referrals for a variety of concerns that may impact the patient’s quality of life, living conditions, or healthcare access and meets with the patient as often as required to determine their needs and refer them to community resources for ongoing support.

Dietitian Program:

  • For patients ages 10 and up with one or more eligible conditions (Crohn’s disease, IBS, ulcerative colitis, celiac disease, kidney health, liver health, or polycystic ovary syndrome) and without coverage by other health payers or financial means to access a dietitian.
  • Provides a one-on-one appointment with one of our dietitians who assesses the patient and connects them to the most appropriate type of care (continued appointment(s) with our dietitians; referral to an AHS, Primary Care Alberta, or community service; self-management resources; or referral to a Primary Care Registered Nurse for patients of members with Enhanced or Comprehensive membership only).

Physiotherapy Program:

  • For patients with rotator-cuff related shoulder pain, joint-related degenerative hip and/or knee pain, or chronic low back pain (back pain patients must be 18 years or older) and without coverage by other health payers or financial means to access physiotherapy.
  • Connects patients with one-on-one physiotherapist appointments, the GLA:D Back program, the GLA:D Hip and Knee Osteoarthritis program, an exercise-based rehabilitation plan, or AHS or community-based resources.

RhoGAM/WinRho:

  • For patients who need a RhoGAM/WinRho injection within 24 hours for a bleed event or miscarriage (please include the necessary ABO/Rh and antibody screen from within nine months with your referral).
  • Patients are seen at the Primary Care Centre, which is open every day and provides encounter notes to you by fax after the appointment.

24-hour Blood Pressure Monitoring:

  • For patients who need 24-hour blood pressure monitoring with a report back to you.
  • Our single referral form provides a choice of four providers with six locations so you can choose the preferred location for you and your patient.
