Membership model

The CWC PCN’s membership model is designed to align with and provide sustainable support to you as you build Patient’s Medical Homes.

The College of Family Physicians of Canada advocates the Patient’s Medical Home (PMH) model as a “vision for family practice across Canada.” Their goals for it include:

  • Team-based care
  • Timely access
  • EMR use
  • Continuous evaluation and improvement work

PMH is also a provincial objective.

The membership model reflects this commitment to the PMH. Membership is inclusive with core programs and services available to all. Your readiness to engage in PMH activities with our PCN determines your membership type.

Information on which programs and services are available for each membership type can be found in the Programs & Services Guide, on related web pages, and outlined in the Mutual Accountability Agreement, which is updated annually.

For physicians who cannot participate in PMH activities with the CWC PCN due to the nature of their practice.

Standard membership icon

Standard membership

For physicians who are not quite ready to build PMHs or are not presently interested in CWC PCN support with completing PMH activities.

Enhanced membership icon

Enhanced membership

For physicians interested in working with support from the CWC PCN on their own PMH improvement goals for their panel of patients.

Comprehensive membership icon

Comprehensive membership

For groups of two or more physicians interested in working together with support from the CWC PCN on shared PMH improvement goals for their panels of patients.

Pediatricians

Pediatricians have access to programs and services appropriate for their practice.
