Updated palliative care pathways

Updated palliative care access pathways for pediatric and adult patients are now available on Specialist Link:

• The adult pathway provides links and referral information for the palliative care consult service and home care.
• The pediatric pathway references the children’s hospice and palliative care service — in addition to consults — and home care.

The pathways, which list routine, urgent and emergent options, break down both urban and rural choices for referring providers.
