Our Senior Services team provides comprehensive support for your patients with age-related conditions and their families and caregivers.
Referrals can be made for:
- Cognitive screening assessments
- Environmental risk assessments
- Fall assessments
- Medication reviews
- And more
Referrals to Senior Services can be made at any time for patients over the age of 65 or physiologically aged:
- We encourage referrals to be made before patient conditions become critical.
- The Senior Services team includes extensive expertise: Nurses, geriatric psychiatrists, social workers, and neurologists.
- This team can also advocate for patients and their family members — taking some of the workload off you — as they navigate various healthcare and community programs.
To refer a patient to Senior Services:
- Provide a detailed referral using the referral form
- Complete the safety section of the referral form
- Consider providing the Senior Services patient handout from our Programs & Services Guide on our programs and services overview web page
Our team will then triage the patient to determine their care plan. The team might see patients in their home, virtually, or in the Primary Care Centre, depending on the patient’s needs.
