Supporting attachment for patients with complex needs

Our PCN has developed an internal process to help patients with complex needs who don’t have a family physician to find a suitable family physician within our network.

Referrals come from various outside agencies, such as AHS and local pediatric initiatives. Physician Liaisons then:

• Identify which physician members would be a good match based on the patient’s needs and location
• Reach out to the members to help these patients find a medical home

We appreciate the compassion and willingness demonstrated by several members who have accepted these patients.
