Senior Services team update

The Senior Services team is fully staffed following a physician-recruitment drive and has reduced their waitlist significantly.

The specialized team provides integrated, team-based care to patients aged 65 or older or physiologically aged via access to physicians, geriatric psychiatry, pharmacy, social work, and nurses certified in seniors health.

All members can:

• Refer their patients to Senior Services, who will triage them to the appropriate team member
• Call Senior Services directly at 403.686.0020 Monday – Friday, 8:30 a.m. – 4:30 p.m. for information about the program, its referral process, or navigating community-based services

Senior Services can:

• Complete in-depth assessments that may include cognitive screening, functional assessment, environmental risk, medication review, and fall or nutritional assessments
• Play a vital role as an advocate for patients who may be isolated
• Expertly navigate the healthcare and community support systems, including sending referrals to AHS Seniors Health when necessary
