With support from their clinic manager and the CWC PCN, the three physician members of Fairmount Medical Clinic went live on the Community Information Integration/Central Patient Attachment Registry (CII/CPAR) at the end of February.
“Now I’m seeing every patient who goes through emergency, every patient who gets admitted, and that information is really valuable and encourages the process,” said Dr. Monica Chu. “I think it’s worthwhile, and I think later on we’ll have a lot more benefits.”
Other positives she found:
- Knowing who her current patients are
- Having up-to-date patient demographics and emails, especially valuable for virtual care
The clinic manager, Margaret Shu, was the clinic’s central contact for the project and had investigated CII/CPAR extensively before engaging with the CWC PCN’s Panel Support Program (PSP) in November 2020.
The PSP team assisted with:
- Completing the program’s toolkit and implementing it (developing the formal paneling process, pulling panel lists for the physicians to review, and confirming panel validation in the EMR)
- Filling out the majority of forms to initiate CII/CPAR and assisting with the others
- Supporting EMR configuration and standardization work for CII/CPAR
- Developing processes for after go-live, including ongoing, monthly CPAR report downloads
- Navigating and answering any questions about the CII/CPAR process
“It requires work and time, but it’s worthwhile,” said Dr. Chu. “I can’t see medicine going into the future, especially virtually, if you don’t have a process going toward integration of information.”
The Panel Support Program is available to support formal, sustainable paneling, and CII/CPAR initiation for members with an EMR-based panel able to grant remote access.
