Reminder: Our Physiotherapy Program accepts referrals for patients:
- With general (non-acute) MSK conditions of the shoulder or osteoarthritis of the hip or knee
- Without coverage by other health payers, financial means, or access to AHS-funded services
Referred eligible patients meet virtually with one of our Physiotherapists who expertly assess their needs in a 30-minute appointment and connect them to the appropriate care:
- In-person, one-on-one physiotherapy appointments
- In-person GLA:D Hip and Knee Osteoarthritis program
- Exercise-based rehabilitation plan developed by our Physiotherapists with the patient for self-management (our Physiotherapists will then follow up with the patient virtually)
- Referral to an external service (AHS or community-based program, such as Alberta Healthy Living Program, etc.)
Language barriers: Our Physiotherapists have access to Translation Services, and our team works with the contracted providers to mitigate language barriers on a case-by-case basis.
To refer:
- Please complete the CWC PCN Physiotherapy Program Referral Form in your EMR (or download the form) that includes the full inclusion-exclusion criteria, fax it to our program, and explain the next steps to your patient.
- Consider providing the patient handout linked on the form — bit.ly/physio-handout — to your patient by writing the bit.ly link on a note or sharing it in other ways.
