PCRN virtual care platform transition: Pomelo to Teams

Effective March 1, our Primary Care Registered Nurses (PCRNs) will begin providing virtual appointments to patients through Microsoft Teams — a change from the previous Pomelo platform.

Transitioning to Teams will improve our operations and enhance efficiency when providing PCRN care.

This change will be seamless for members and patients:

• There will be no impact on past or future referrals to PCRNs.
• Patients who had Pomelo appointments booked after March 1 have been provided with a new Teams link. They will also receive a Pomelo cancellation notification (if applicable), and a phone call from our team to confirm they received the new link.

This update will allow us to continue to provide patients with accessible, virtual nursing care.
