Our Health Information Coordinators (HICs) and Patient Care Coordinators (PCCs) work collaboratively to support screening and outreach projects. These screening programs help catch medical concerns early — ultimately improving health outcomes for your patients.

What if you wanted to begin a breast cancer screening project and had an assigned HIC-PCC pairing?

  • First, your PMH team would meet with you to discuss your screening goals. Your HIC will review your panel data and make recommendations for projects that would be a good fit for your panel. You can also select projects that are of interest to you (e.g., improving breast cancer screening rates by 20 per cent).
  • After determining you would like to move forward with a breast cancer screening project, your HIC will use the Alberta Screening and Prevention recommendations (ASaP) as a guideline for determining which patients are due for the mammogram.
    • According to ASaP, patients aged 45 – 74 should receive a mammogram every two years.
  • Your HIC will then pull a list of all patients due for a mammogram, then use EMR data and Netcare to ensure they have not been screened elsewhere.
  • Your PCC will then reach out to patients due for a mammogram to ensure they receive their screening.
  • Your HIC will track the progress of your projects in Monday.com and meet with you after four – six months to check in and provide project updates. A Monday.com user guide is available on our website for reference.

We regularly review CWC PCN patient demographic statistics and provincial benchmarks to set both internal screening targets and personalized targets based on your unique goals and objectives.
