Our Primary Care Registered Nurses (PCRNs) are here to support patients with various concerns — including nutrition management.
Using an individual with diabetes as an example of nutrition management, the PCRN would first:
- Work with the patient to understand what success looks like to the patient so they can tailor their support accordingly.
- Determine the patient’s existing knowledge about nutrition and diabetes, fill any knowledge gaps, and dispel any misconceptions — ensuring the patient is equipped with accurate information.
- Explore the factors that influence the patient’s health — the patient’s home environment, cultural background, and other circumstances — to provide guidance that is both relevant and practical to the patient.
In initial or following appointments, the patient and PCRN would work together to:
- Determine a nutritional plan that works for the patient, with the PCRN supporting them in planning meals and using patient information, such as recorded blood sugar levels or medication history, to make recommendations for diet or medication adjustments.
- Make relevant referrals, as appropriate; the PCRN may recommend a referral to our Dietitian Program for additional support or encourage the patient to participate in screenings or tests associated with their condition.
- Provide support with an empathic, patient-centred approach to help the patient take control of their nutritional health and eliminate any overwhelming or negative feelings.
Members who have an assigned PCRN can make referrals for nutrition management directly to them.
Referring a patient for a nutrition management appointment can assist them with building healthy habits that greatly contribute to overall wellness.
