Our robust Senior Services team provides integrated, team-based consultative care to patients aged 65 and up, or physiologically aged, via access to physicians, geriatric psychiatry, pharmacy, social work, and nurses certified in seniors health.
One referral will connect your elderly patient with the team who will:
- Triage the patient to the appropriate parts of the team with the number of visits and team members involved based on the patient’s needs
- Send you a consultation note after each encounter
When referring a patient, please:
- Provide a detailed referral on the referral form to help the team triage efficiently
- Complete the safety section of the referral form
- Consider giving the patient or their caregiver the Senior Services patient handout from our Programs & Services Guide
