For patients needing translation assistance to access nutritional support, help is available:
- Our Primary Care Registered Dietitians and Primary Care Registered Nurses both have access to Translation Services.
- Health Link’s dietitian self-referral form includes a field to notify them that the services of an interpreter are required and the language needed (they recommend a healthcare team member submit the form on behalf of the patient).
- Patients calling Health Link at 811 can remain on the line without selecting an option to be connected with a staff member who can bring in an interpreter.
Our Dietitian Program provides support for your patients ages 10 and up who are otherwise unable to access one-on-one dietitian support for:
- Gastrointestinal health (IBS, Crohn’s disease, ulcerative colitis, and celiac disease)
- Liver health
- Kidney health
