Social Work program: Connect your patients with key resources

Our Social Workers help bridge the gap between your patients and community supports — increasing their understanding of the healthcare system and access to key resources.

This support could include:

• Support in accessing funding
• Help with accessing housing assistance
• Support in understanding their journey through the health system
• And more

Referrals to this program can be made for patients who are, or will be, experiencing concerns impacting their quality of life, living conditions, or healthcare access.

To help set patients up for success in the Social Work program:

• We encourage referrals to be made before patient circumstances become critical.
• It is helpful for patients to enter the program with realistic expectations for support; our Social Workers cannot guarantee patients will qualify for certain programs or funding.

Social Worker appointments may be conducted over the phone, video, or in-person depending on the patient’s needs.

All members can refer their patients to the Social Work program through the CWC Primary Care Centre referral form.
