We have launched our new online 2023-24 Annual Report for our members, the public, and our teams.
For our members, the annual report highlights many of our efforts to support you and your patients over the last fiscal year:
- To support members with QI and practice improvement, we launched a support project to assist you with the College of Physicians & Surgeons’ requirements for practice improvement.
- To develop our new Dietitian Program and Physiotherapy Program, we consulted with a diverse range of individuals and organizations, including our membership and AHS and community-based partners.
- To increase access, we expanded the hours of the Primary Care Centre and our Mental Health Program.
Physician members are the foundation of PCNs, and we appreciate your work with patients and engagement with our teams throughout the last year and beyond.
Additional articles share more about our work in PCN priority areas:
- Patient’s Medical Home (PMH): Assisting with CII/CPAR, supporting lung cancer referrals, and how our nurses enhance their clinical expertise
- Community Services, Transitions, and Integration (CSTI): Creating the single referral form for 24-hour blood pressure monitoring and working with our Patient Advisory Council
- Member Services & Engagement (MSE): Enhancing engagement with members, offering CME & PD opportunities, and improving communications to the public
