24-hour BP monitoring update

Thanks to feedback collected in a survey of referring members, we learned a small number of physicians were not receiving the 24-hour blood pressure (BP) monitoring reports as they should and immediately reached out to the providers.

The issue was with the technology used by three providers, and the biotechnology company behind it has resolved it.

Refresher: All members can refer their patients for no-cost 24-hour BP monitoring to their choice of four providers with six locations using our single, combined referral form:

• Advanced Cardiology with two locations
• Advanced Respiratory Care Network with two locations
• C-diagnostics (a division of C-health) with one location
• Core Pharmacy with one location

To refer, please complete the referral form in your EMR — search for “CWC PCN 24h BP Referral Form” — (or download the PDF referral form) and fax it to the location of your choice.

Acting on member feedback:

• We first reintroduced 24-hour BP monitoring with a form for Core Pharmacy’s no-cost service and made members aware of the three other providers’ no-cost service.
• We then rolled out a single, combined referral form for all four providers, developed through collaboration.
• Most recently, we raised the issue discovered through the survey of referring members to providers so it could be resolved.
