Combined 24-hour BP monitoring referral form

We heard you! Presenting our new 24-hour blood pressure (BP) monitoring referral form you can use to refer to four service providers with six locations in Calgary:

• Advanced Cardiology with two locations
• Advanced Respiratory Care Network with two locations
• C-diagnostics (a division of C-health) with one location
• Core Pharmacy with one location

Many members suggested a single form, so we collaborated with the different providers to create one.

Thank you for sharing your feedback so we could make improvements. With the single form, you can now quickly refer to the location most convenient for your patient.

Please download the new referral form and upload it to your EMR. Where possible, our Health Information Coordinators are replacing the old form with the new one in clinic EMRs (search “CWC PCN 24h BP Referral Form”) to save you time.

When referring, please ensure you fax the form to the specific fax number of the location of your choice (all fax numbers are on the form).

All the listed options for 24-hour BP monitoring on the form provide:

• 24-hour blood pressure monitoring with no cost for your patients
• A report back to you with interpretation of the results
• Additional capacity to meet your need for 24-hour BP monitoring
• The ability to book urgent referrals when indicated

Wait times vary based on referral volumes and patient availability but are typically two – four weeks.
