The Diabetes, Obesity & Nutrition Strategic Clinical Network (DON SCN) is a diverse group of patients and healthcare stakeholders who develop initiatives to improve care for these three interconnected areas of health.

Projects and initiatives of use in primary care:

  • The Diabetes Foot Care Clinical Pathway has tools and resources to support providers and patients in the early identification of foot problems to prevent diabetic foot ulcers and related amputations.
    • New to the pathway is the diabetes foot health self-screening tool that guides patients in checking and caring for their feet at home.
  • The Diabetes Eye Care Clinical Pathway has resources to improve provider and patient awareness about the importance of screening for diabetic retinopathy and increasing screening numbers.
  • The Virtual Diabetes Prevention Program is a virtual program providing personalized, one-on-one support for Albertans with prediabetes through a connection with a health coach.

Eligibility

  • The Diabetes Foot Care Clinical Pathway is for patients with known diabetes; please consult the Diabetes Foot Care Pathway on specialistlink.ca for referral options based on pathway outcomes.
  • The Diabetes Eye Care Clinical Pathway includes a referral form to optometrists and is for:
    • Type 1 diabetes: Children aged 15 and up with diabetes for five years should be screened annually
    • Type 2 diabetes: Children, adolescents, and adults should be screened at diagnosis and then annually
    • Pregnant patients with Type 1 or Type 2 diabetes: Screen before conception, during the first trimester, as needed during pregnancy, and within the first year of postpartum
  • The Virtual Diabetes Prevention Program targets Albertans with prediabetes (aged 18 and up, A1C range 6.0 – 6.4, and BMI of 25 or more); please consult the primary care guide for how to refer.
