Updated Primary Care Centre referral form

We have revised the Primary Care Centre referral form in alignment with the new options for 24-hour blood pressure monitoring.

All members can use the form to refer patients to our:

• Access Appointment Service
• Senior Services
• Social Workers

The updated form provides:

• Nearly double the space to specify the reason for referral
• The option to include a patient’s preferred pronouns

Please download the referral form and add it to your EMR.

Work is underway so that, where possible, our Health Information Coordinators will be able to add it to clinic EMRs.
