New no-cost 24-hour blood pressure monitoring

In consultation with our Medical Director, our PCN is partnering with Core Pharmacy where the pharmacy will provide referral-based 24-hour blood pressure monitoring in place of the Primary Care Centre.

We know many members have been asking about 24-hour blood pressure monitoring, and this new development means:

• All members can refer patients to Core Pharmacy for 24-hour blood pressure monitoring without cost and minimal wait time.
• Referring members will receive a report that includes interpretation of the results.
• The Primary Care Centre can focus on providing appointments for the high volume of referrals to the Access Appointment Service, which continues to surpass pre-pandemic levels, instead of a duplicate service.

Please download the new referral form (form has been updated with multiple providers) and upload it to your EMR. Where possible, our Health Information Coordinators are adding it to clinic EMRs (search “CWC PCN 24h BP Referral Form”).

Core Pharmacy does not charge a fee for this service and has:

• The capacity to meet our members’ need for 24-hour blood pressure monitoring (which the Primary Care Centre does not due to the Access Appointment Service referral volume)
• Newer monitoring equipment than the Primary Care Centre
• Technology to provide interpretation of the results
