Community Connect YYC

In 2021, we partnered with Community Connect YYC to give our members and CWC PCN regulated health professionals the ability to book affordable or no-cost counselling appointments for patients through one online platform.

All members can use the booking tool by selecting “CWC PCN physician member” in its agency drop-down menu:

Community Connect YYC connects patients to affordable counselling services from multiple agencies that provide:

• Rapid access counselling (within three business days)
• Adult general counselling
• Counselling for children and families and for youth and families
• Counselling for persons with disabilities
• Sexual health counselling
• Domestic violence support
• Counselling for immigrants and those with first language needs
• Peer supports or caregiver supports

Services are offered at no cost, on a sliding scale, or on a model that allows patients to choose their own session fee. Every participating agency ensures that finances will never get in the way of patients accessing their services.

It is sometimes difficult for patients to take the first step on their own. With Community Connect YYC’s online booking tool and consent from your patient, you can:

• Select the most appropriate service
• Directly book an appointment with the service for your patient, usually within a week
• Know when their initial appointment was booked for so you can follow-up as needed*

*Please note: the availability of post-appointment information for the referrer varies by service, and patient preference, and the appointment confirmation only goes to the patient.
